Form I-94 (Arrival/Departure Record)

Form I-94 (Arrival/Departure Record) is the white card issued to an individual when s/he enters the U.S. This document is also attached to the bottom portion of a Notice of Action (Form I-797). The Form I-94 states an individual’s immigration status and its expiration date.

The approval notice sent to an individual (Notice of Action Form I-797) includes a new I-94 card (also known as an Arrival/Departure Record), evidencing immigration status. The individual should attach the bottom right portion, Form I-94, of the original Approval Notice to the initial Form I-94 (the white card stapled in the passport).

Upon leaving the country, individuals are asked to hand in their Form I-94 cards. Individuals should have both the white Form I-94 and the bottom right portion of the original Approval Notice Form I-94. Individual should keep the left bottom portion of the original Approval Notice in their passports (not relinquishing it with the white Form I-94).
